This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) Current, full, active, and unrestricted registration as a graduate professional nurse AND nurse practitioner in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Possess a master's degree from a program accredited by the NLNAC or CCNE and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body. Preferred Experience: 5 years of primary care or internal medicine experience preferred Geriatric certification or geriatric experience preferred Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. ["VA offers a comprehensive total rewards package: VA Nurse Total Rewards The Nurse Practitioner for the Home Based Primary Care Program is an Advanced Practice Nurse with clinical experience in the care of adults across the life span. The HBPC mission is to provide comprehensive, interdisciplinary, primary care in the homes of Veterans with complex medical, social, and behavioral conditions for whom routine clinic-based care is not effective or may not be possible. Home based primary care provides interdisciplinary care that is longitudinal and comprehensive rather than episodic and single-problem focused. The Home Based Primary Care NP reports directly to the Deputy Chief of Primary Care. Additionally, the HBPC NP works directly with the Chief for Coordinated Community Care for operational issues and consults with the HBPC Medical Director for clinical issues. The Home Based Primary Care NP: Assumes primary medical responsibility for assigned Veterans. Participates in the determination of the appropriateness of Veterans for admission to and or discharge from the HBPC Program. Assesses medical problems and defines medical management Participates in the HBPC interdisciplinary team by working interdependently in assessing, planning, problem solving, decision-making, and implementing team tasks. Initiates an individualized plan of care through the initial team care planning process. Reviews progress towards goals at regular intervals, as a team, on a quarterly basis, or more frequently when there is a change in the Veteran's condition. Serves as a role model for effective operation of an interdisciplinary team. Completes home visits for clinical assessment, treatment, plan of care, and interdisciplinary purposes. Collaborates with Veterans and caregivers in care planning. Assesses and supports the caregiver's capability to continue to provide care. Provides education to the Veterans and caregiver. Determines the need for subspecialty clinics. Determines the need for and facilitating admission to the hospital. Consults with the HBPC Medical Director as needed. Responsible for reviewing clinical documentation from non-VA entities. Identifies appropriate Veterans for HBPC admission, provides continuity of care across settings, integrates HBPC with non-VA care, expands rural access, integrates telehealth, improves quality of care, meets care needs through the end of life, and incorporates home care into medical/clinical education and research. Enhance the Veterans' quality of life through the end of life by respecting the needs and preferences of the caregiver, supporting the option of dying with palliative care at home if that is the Veteran's wish. Provides palliative and end of life counseling and life sustaining treatment planning. Maintains compliance with and supports HBPC-specific performance measures, external peer review program (EPRP) standards, local policies, standard operating procedures, and Joint Commission standards. Participates in developing and implementing quality improvement and Veteran safety practices. Drives a government vehicle to complete HBPC home care services. In addition to being a care member of the HBPC interdisciplinary team, the HBPC NP may also perform nurse functions, to include care management, delivering nursing care in the home setting, linking Veterans with resources, and coordinating resources for comprehensive care to avoid duplication of services. Provides support when needed for Traveling Veteran Coordinator tasks and duties. Provides support when needed for Non-VA Care tasks and duties. The HPBC Nurse Practitioner will practice according to the Standards of Practice for Advanced Practice Nurses, with Full Practice Authority as defined by the VHA Directive 1350, and according to the approved clinical privileges.
