SummaryThe Inpatient Care Coordinator (ICC) is responsible for the seamless transition of patients from the acute to the most appropriate post-acute level of care. ICCs provide inpatients with education on disease process - inpatient tests - and discharge instructions. Assist providers with placement of consults and review charts to ensure there are no barriers to a timely and safe discharge.
