DescriptionPOSITION SUMMARY:  Responsible for managing, coordinating and integrating all Care Coordination services within CHRISTUS St. Vincent Health System.  Provides Care Coordination services to patients based on person-centered acuity and coordinates patient in hospital or clinic setting.
