Summary: The Clinical Referral Processor will review all referrals to ensure the care needs can be met safely by the Home Health Agency. Work with the referral source to ensure that all clinical information is present to schedule the patient for admission to the agency.
Other information:
Basic ICD 10 coding experience preferred.
Computer skills required.
Ability to form relationships with referral sources to ensure a smooth and timely transition of care.
1 weekend day rotation shared with other Intake staff
Responsibilities:
Review referrals to the agency to ensure they are appropriate for the agency.
Review for appropriate ICD 10 codes to met regulatory requirements for Home Care.
Review referral for all required information needed to transition from referral source to Home Care (wound care orders, medication list, discharge summary)
Along with data entry staff, ensure all referrals are entered in the EMR.
