The Patient Flow Coordinator is responsible for ensuring the patients admitted to the VA Palo Alto Health Care System (VAPAHCS) meet the requirements for the appropriate level of care for the specific treatment plan regardless of location or services, collaborates with Utilization Management as a benchmark, ensuring patient is meeting criteria for admission and or continued stay. Is clinically and administratively responsible to the Nurse Manager of the Clinical Command Center. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
