The Registered Nurse Hospice and Palliative Medicine (HPM) Case Manager is a member of the Hospice and Palliative care section, Geriatrics and Extended Care service. The incumbent is responsible for identifying and facilitating options and services for meeting complex patients' health needs while decreasing fragmentation and duplication of care and enhancing quality focuses on managing pain, symptoms, and resources for palliative care/hospice Veterans in the inpatient and outpatient settings. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
