The Licensed Practical Nurse (LPN) is part of the Patient Aligned Care Team (PACT) that provides clinical and technical support to patients receiving care at the Northwest Community Based Outpatient Clinic. The incumbent works independently under the direction of the PACT Registered Nurse (RM) and reports to Assistant Nurse Manager (ANM) or the Clinic Nurse Manager (CNM). Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Grade Determinations:GS-3 None beyond the basic requirements. LPN is expected to perform all duties within scope of their license. However, completed work may require routine work or detail higher level review depending upon the complexity of the duties involved. GS-4 Experience: At least (6) six months of qualifying experience as an LPN or LVN. OR Graduation from an approved school and have (1) one year of experience that involved nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure and sterile techniques and procedures. OR Graduation from an approved school of at least 24 months duration. GS-5 Experience: Completion of at least one year at the GS-4 level or equivalent of qualifying experience as an LPN and can demonstrate the following: Demonstrated knowledge and ability to provide a full range of practical nursing care to patients with a variety of physical and/or behavioral problems. [Works with the RN and/or MD/DO to appropriately orient/train less experienced LPNs/LVNs and/or Nursing Assistants (NAs)/Health Technicians (HTs) in relation to acceptable standards of practice in promoting optimal patient care delivery. Provides effective education to patients and/or family members in relation to common disease processes, medication, and /or prescribed treatment regimes. Demonstrated ability to serve as a responsible member of the nursing team and interact in an appropriate manner with patients, family members, professional and [other] supportive personnel involved in the delivery of] patient care, [incorporating acceptable, established customer service standards into practice. Knowledge and skill sufficient to prepare, administer, and appropriately document actions taken specific to commonly prescribed oral, topical, subcutaneous, intramuscular, and/or intravenous medications as permitted by approved local facility policies and procedures. Observation and documentation will include patient's response to medication administered and the reporting of any noted change in patient's condition to RN or MD/DO. Knowledge and ability to recognize the need for and to institute emergency measures when indicated, promptly seek the assistance of the RN or MD/DO, and assist in resuscitation procedures in cardiac and/or pulmonary arrest. Recognize and appropriately responds to breakage/malfunction or loss of equipment, safety hazards, and supply deficiencies, promptly reporting to appropriate personnel for corrective action. GS-6 Experience: Completion of at least one year at the GS 5 level or equivalent of qualifying experience as an LPN or LVN, fully meeting all performance requirements for the GS 5 LPN and am technically proficient in initiating, performing and completing assigned duties in providing care to variable patient populations. Completed work needs only a general review by a registered nurse (RN) or physician (ND/DO) for appropriateness and conformity with established policies/procedures. Preferred Experience: GS-06. Physical Requirements: Heavy lifting (45 lbs and over); Heavy carrying (45 lbs and over); Straight pulling and pushing; reaching above shoulder; use of fingers; both hands required; walking and standing up to 8 hours; repeated bending; Ability for rapid mental and muscular coordination simultaneously; hearing aid permitted. ["The Licensed Practical Nurse (LPN) is responsible for observing, recording, and reporting status and changes in the patient's condition as well as assisting in the development of the patient-driven holistic care plan. The Incumbent collects information from the patient and documents appropriately in the patient's electronic medical record; and performs functions as support for the provider assigned to a PACT, primarily in assisting with clinic patient flow, patient history and completion of nursing focused clinical reminders and alerts provider to any concerning findings. The Incumbent works under the direction of the provider or RN for appropriateness and conformity to policy and accepted nursing practice standards. The incumbent manages patients from young adult to geriatric patients and functions independently within the LPN scope of Practice. And anticipates possible medical procedures or testing based on critical awareness and observation discovered during patient intake. The LPN assist the provider with examinations or procedures as needed. The incumbent will support operations in the lab collection area providing phlebotomy and other lab specimen collection processes. The incumbent possesses administrative, clinical and technical skills and performs other administrative duties as assigned to support patient and clinical needs and to minimize scheduling conflicts.
