These HBPC RN positions are located in Geriatrics and Extended Care Service at the VA Togus, Augusta, ME and the Bangor CBOC locations. These positions are full-time at 40 hours per week. Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. HBPC provides a patient centered approach to care and treatment in the patient's home. The HBPC RN functions as a care manager for the HBPC assigned Veterans and is responsible for continuous assessment of the Veteran and caregiver needs. Responsibilities include, but are not limited to: Accurately performs assessments/screens for: nutrition, pain, tobacco use, alcohol, spirituality and depression and SLUMS Coordinates outcome focused interdisciplinary care plans that reflect the team's assessments, interventions, evaluations and follow-up. Implements appropriate nursing/team plans of care reflecting the patients' functional capacity changes, living arrangements, financial resources and support systems. Demonstrates ability to care manage HBPC clients through the timely completion of screening tools, referrals to other services, coordination with VA/Community providers and assessment and documentation of patient/family education. Administers medications following established Medical Center and Service Line Policies and Procedures. Assesses patients for abnormal or non-therapeutic reactions to these medications and initiates appropriate interventions. Demonstrates ability to do wound care according to treatment orders. Educates caregiver in techniques for turning and positioning of patient. Demonstrates ability to train patient/family in suctioning/care for NG tube. Demonstrates ability to integrate telehealth technology into practice. Applies evidence-based standards to screen, immunize and promote health activities in the HBPC patient. Demonstrates the ability to perform phlebotomy in the home. Transport blood draws to the Lab. Maintains government vehicle and mileage reports. Accurately completes required workload reports.
