The PACT - Registered Nurse (RN) Care Coordinator reports to the primary care nurse manager. The PACT RN Care Coordinator demonstrates leadership and mastery of practice in delivering efficient, comprehensive, and continuous patient driven holistic care through active collaboration, communication, and coordination of resources. The PACT RN Care Coordinator works in partnership with the assigned primary care provider and a panel of patients, for effective disease and chronic care management.
