The Nurse Practitioner (NP) - Patient Aligned Care Team (PACT) Provider is under the administrative and clinical supervision of the Regional Service Chief. The NP is responsible for self-directed delivery of patient care, assuring s/he works within approved clinical privileges. The NP is also responsible to the Associate Director, Patient Care Services for ensuring the Nursing Standards of Patient Care and Nursing Practice are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. On and after [March 17, 2009], registered nurses appointed or otherwise moving into these assignments must meet and maintain the following additional qualifications. This includes employees appointed before [March 17, 2009], who obtain such qualifications on or after [March 17, 2009.] Nurse Practitioners must be licensed or otherwise recognized as a nurse practitioner in a State, possess a master's degree from a program accredited by the ACEN or CCNE, and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body. The certification must be in the specialty to which the individual is being appointed or selected. Preferred Experience: Completed a residency in Primary Care or Family Medicine Minimum of 5 years practice as a NP in Primary Care or Family Practice Doctor of Nursing Practice (DNP) Unrestricted DEA license Prior VHA experience Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level III - Master's degree in nursing (MSN) and no additional nursing practice/experience. Nurse II - Master's degree in nursing and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. Mechanical inpatient lifts are provided. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Duties include but are not limited to: The NP demonstrates performance and leadership that is broad enough to care for a group of patients. The NP is an independent member of the Medical Staff with delineated clinical privileges. The NP is accountable for the evaluation, management, and coordination of patient driven care. S/he provides comprehensive outpatient primary, urgent care, preventive, and wellness care as well as a spectrum of acute, episodic, and end-of-life care to Veterans. This includes performing history and physicals, ordering and interpreting diagnostic and therapeutic test/studies, initiating consults and referrals, prescribing medications and supplies in accordance with VA policy, VA formulary and State regulations, and formulating, initiating, and evaluating healthcare teaching for primary care patients and their families. The NP is responsible for documented clinical performance outcomes at the primary care service level. S/he guides, develops, and supports staff from a leadership perspective. S/he uses professional standards of care, scientific evidence, and practice to evaluate programs and/or service activities. The NP will be a primary clinical surrogate for licensed independent provider(s) at their assigned duty station. The NP will work from a designated duty station and will be responsible for providing comprehensive primary care services to a defined panel of patients or other patients as, assigned. This includes history taking, physical examinations, ordering appropriate labs and diagnostic tests, needed consultations and formulation of treatment plans within the scope of their national certification and facility credentialing. Documentation is via an electronic record and prescriptions according to guidelines of the VA formulary. A patient centered approach with emphasis on preventative strategies and patient education, plus flexibility in providing care on urgent or same day basis and via telephone visits are required. The NP is part of a Patient Aligned Care Team (PACT) in which nurse care managers, support staff, and other professionals all work together to provide seamless care process which ensures efficiency, customer service, and clinical performance according to evidence based standards of medical practice for providing primary care to Primary Care patients at a primary care clinic. The NP is responsible to contribute to the achievement of quality of care standards, patient access, efficiency, process improvement, and patient satisfaction within the Ambulatory Care Services, in particular to the patients that are within the NP 's assigned patient panel. The NP may be temporarily assigned other clinic duties as a provider, temporarily or permanently, within the service (i.e. float/gap coverage) where coverage is needed or may be assigned to conduct telehealth visits. If a temporary location is outside of reasonable commuting distance (greater than 50 miles from assigned duty station), the NP will be reimbursed for commuting mileage or lodging/per diem expenses.
