There is a nationally recognized initiative to seamlessly transition the health care of injured and ill Servicemembers and Veterans from the Department of Defense (DOD) to the Department of Veterans Affairs (VA) health care system. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: Ability to expand clinical knowledge in the profession, provide consultation and guidance to colleagues, role model effective nursing practice skills, teach or provide orientation to less experienced workers, and develop innovations in practice interventions. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. The initiative is led by the VA Central Office, Care Management and Social Work Services, Office of Patient Care Services (10P4C). The VA Liaison for Healthcare (hereinafter referred to as VA Liaison) is seen as the Veterans Health Administration (VHA) representative to the military installation and must represent the VA in all aspects of patient care, transfer and outreach. The primary role of the VA Liaison is to ensure the transfer of health care, both inpatient and outpatient, from the Military Treatment Facility (MTF) at Ft. Drum, NY to the appropriate VA health care facility. The VA Liaisons must work on-site at the MTF with staff, Servicemember(s), families, Caregivers, and Veterans to ensure priority access to needed health care services and education regarding VHA benefits. Servicemembers and Veterans returning from combat theaters may have severe and complex illnesses and injuries, minor illnesses and injuries, and mental health needs. Although the VA Liaisons must report administratively to the VA health care facility closest to the MTF, they must report programmatically to the VHA Care Management and Social Work Services, Office of Patient Care Services (10P4C). The practice of each VA Liaison is based on knowledge, experience, and research, and is expected to impact patient outcomes and improve care coordination and continuity for polytrauma patients, as well as for returning Servicemembers and Veterans with both severe and non-severe injuries. The VA Liaison is accountable for clinical program effectiveness and modification of service patterns. The VA Liaison is also assigned in a setting where there is no direct access to social work supervision and is assigned to work with special patient populations with highly complex health and mental health problems. Working closely with the MTF treatment team as an independent practitioner; using advanced practice skills and expertise to provide ongoing consultation regarding complex discharge planning issues; and VA health care benefits, resources and facilities. This requires an intimate knowledge of VHA programs and services nationwide, and the ability to match Veterans' needs with appropriate resources. Clearly identify the Servicemember's/Veteran's health care and psychosocial needs and include requests for VHA health care services to ensure Clinical and Consult Orders or authorizations, specifying which VHA services are authorized and are completed prior to the transfer of any Servicemember(s)/Veteran(s) to a VA health care facility.\nMeeting with the Servicemember/Veteran and family to provide education and an overview of VA health benefits and resources, and to address current medical and psychosocial issues identified as part of the Servicemember's/Veteran's treatment plan. In collaboration with the MTF treatment team, the Liaison must use advanced clinical skills to assess the Servicemember/Veteran and family's psychosocial situation, their ability to comprehend and comply with VA's treatment plan determined by the MTF staff, and any special needs of the Servicemember/Veteran and family that may impact reaching optimal psychosocial functioning.
