The Home Based Primary Care (HBPC) Program Director/Nurse Manager is a member of the Geriatrics service. S/he is responsible for the assessment, planning, coordinated provision, and evaluation of interdisciplinary services to Veterans in the HBPC Program. S/he is responsible for directing and organizing program specific orientation, training, and the development of the care team. S/he is responsible for directing, supervising, and evaluating staff performance.
