The Licensed Practical Nurse (LPN), functions as a member of the team as assigned. Demonstrates sound techniques in complex nursing care procedures and basic care of patients in all areas. The Community Living Center provides Veteran centered care using an interdisciplinary team approach to provide a full range of nursing care to the adult & geriatric resident. Services provided within the Community Living Center include dementia memory care, geriatric evaluation & management, wound care, CARF accredited acute rehabilitation, long-term antibiotic care, respite care, restorative care, skilled nursing & rehabilitation, mental health recovery & long term care. The Community Living Center provides Veteran Centered Care using an Interdisciplinary team approach to provide a full range of nursing care to the adult and geriatric Resident. We seek LPNs who possess good organizational skills which enable completion of work and ability to effectively communicate with Veteran Residents, families, and other members of the Interdisciplinary team. The LPN should have a working understanding of cultural transformation, a home-like environment, and a dedication to improving the quality of life for the Veterans and providing end of life support for Veterans and/or significant others during the time surrounding death. Incumbent completes work with only a general review by an RN. Demonstrated ability to respond to the patient's nursing needs for nursing, medication, and equipment-assisted care; and to recognize deviations from normal. Demonstrated ability to work effectively with patients, family and other staff members and to respond appropriately to emergency situations. Incorporates acceptable, established customer service standards into practice. Demonstrates knowledge and skill sufficient to prepare, administer, and appropriately document actions taken specific to commonly prescribed oral, topical, subcutaneous, intramuscular, and/or intravenous medications as permitted by approved local facility policies and procedures. Observation and documentation will include patient's response to medication administered and the reporting of any noted change in patient's condition to RN.
