This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Learn more The Caregiver Support RN provides clinical evidence-based services and interventions for the caregiver and Veteran; education and consultation for the caregiver and Veteran, on caregiver issues; community outreach; resource development; continuous quality improvement activities; and the evaluation of caregiver issues. In addition, the incumbent, must be a skilled communicator capable of developing effective partnerships with local, state and federal agencies as well as non-governmental organizations. This position is in Mental Health Nursing Services and administratively reports directly to the Nurse Manager of the Outpatient Mental Health Clinics. CSP RN will support the veteran and his/her family by matching services for which they are eligible for. Conduct modified home visits of veteran/caregiver to ensure everything they need to be safe and successful in a home setting. Collaborate with the multidisciplinary team to provide services which may include: Adult Day Health Care, Home Based Primary Care, Skilled Home Care, Homemaker and Home Hospice Care. Function in support of caregivers, Veterans, and family members, throughout medical center and community setting. Responsible for assisting with developing and establishing linkages and collaborate on VA caregiver efforts with community healthcare and social service agencies. Assist with clinical and daily responsibilities of their assignment in the program, as well as, for the management of data related to the program at the facility level. Accountable for clinical and program development and effectiveness of the Caregiver Support Program. Apply advanced knowledge and understanding of the services and benefits available from federal, state, and private sector resources. Perform duties of an oversight RN such as wellness visits or Assessor RN that reviews eligibility of dyads for admissions or continued enrollment within the Comprehensive Program for Caregiver Support.
