The License Practical Nurse (LPN) is located within Community Living Center at the H.J. Heinz Campus Long Term Care Facility. The LPN provides a full range of practical nursing care to patients with a variety of physical and behavioral problems. The LPN serves as a leader and mentor to new LPNs, Nursing Assistants, or Healthcare Technicians. Apply nursing process to administer holistic patient care. Administer medications via all routes, and carries out basic treatments as ordered, monitor patient response to medications and treatments, report changes and document effectiveness. Function as member of the team by participating in planning, implementation and evaluation of nursing care using a focused assessment to care for the adult patient with a variety of physical and/or behavioral problems. Participate in providing or reinforcing effective patient education to both patient and their family in relation to disease process, medication and treatments, procedures and surgeries, equipment and devices or any education deemed necessary. Document the effectiveness of education. Assist the patient in activities of daily living toward the attainment of rehabilitative/restoration goals. Assess life threatening conditions and initiate and/or assist with CPR or other appropriate interventions including prompt notification of an RN and MD/DO to provide appropriate direction and intervention. Adhere to Medical Center policies and procedures when caring for patients on suicidal codes or in restraints or in seclusion. Respond to breakage-malfunction or loss of equipment, safety hazards or supply deficiencies and reports to appropriate personnel for corrective action. Collaborate, communicate, set priorities and organize assigned work and delegate appropriate tasks/responsibilities in order to expedite patient care while ensuring compliance with established procedures, policies, and regulations. Participate in unit-based improvement projects that ultimately improve patient care and/or patient outcomes.
