This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Learn more The Licensed Practical Nurse performs a wide range of resident care and supportive duties in the Community Living Center (CLC). The LPN GS-7 must demonstrate an exceptional degree of practical nursing knowledge and skills necessary for concentrated effort and self-direction to provide care of the medically complex veteran population. He or she must exhibit independent technical judgment, critical thinking skills, and the use of independent judgment to carry out their assigned duties, which include ability to prioritize and coordinate the care of medically complex veteran population with minimal supervision. Such individuals may serve as leaders or co-leaders of a nursing unit for a specific group of residents. Major duties for the position include (but are not limited to): Maintaining a safe environment using appropriate equipment, devices, and safety measures for all residents with demonstrated special awareness of the needs of all residents with impaired mobility; Contributing to the evaluation of fall risk residents and implementing appropriate preventative measures; Incorporating current resident care data by gathering data and actively contributing to the development and modification of treatment plans; Assisting the RN and/or provider to appropriately prioritize overall resident care needs and adjust plans of care delivery; Communicating calmly, courteously, and tactfully with all residents, personnel and visitors maintaining confidentiality in accordance with HIPPAA regulations and medical center policies; Identifying and encouraging participation in the rehabilitation aspect of resident care, such as re-motivation and other resident therapy groups; Conducting routine rounds throughout the shift on a designated group of residents; Evaluating and observing residents level of acuity and communicate/report changes in acuity level to the covering RN and/or provider; Supporting the Nurse Manager and/or Assistant Nurse Manager in monitoring the practice of licensed and unlicensed nursing staff in resident care delivery; Assisting the RN, MDS Coordinator, and other members of the interdisciplinary team with providing input in the development of the care plan based on the resident care needs; Performing all types of dressing changes, including complex wound care; Documenting data in the medical record following established guidelines including identifying the designated RN as a co-signer to notes entered in CPRS.
