This position is within Nursing Outpatient Mental Health, Thunderbird Community Based Outpatient Clinic (CBOC), Phoenix VA Healthcare System (PVAHCS), Phoenix, AZ. Phoenix VA Health Care System (PVAHCS) is a Level 1a major interdisciplinary teaching facility with functional service lines serving medical, surgical, mental health, and long-term care inpatients and outpatients with an Emergency Department and multiple community-based outpatient clinics (CBOCs). The Outpatient Mental Health RN-Primary Care Mental Health Integration Care Manager practices as an RN in many outpatient settings within PACT, Mental Health and the Nursing Service. The RN is accountable for assessing, monitoring, supporting, educating, and effectively coordinating the care of the Mental Health Veteran with other mental health and primary care providers within the system. The RN provides telephone, telehealth, unscheduled and scheduled appointments to Veterans as a support to the PCMHI LIP staff members and to PACT members. The RN completes Clinical Reminders and Suicide Assessments as appropriate. Care is provided according to policies/procedures, Clinical Skills, competency assessments, RN will utilize both CPRS and Behavioral Health Labs (BHL) software in their role. The Primary Care Mental Health Integration Care Coordinator - RN provides care and supportive counseling of persons in the promotion and maintenance of health and prevention of illness and injury, based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses. The RN's primary commitment is to the unit/department as a whole and the patient whether an individual, family, group, or community (ANA Code of Ethics for Nurses). The RN provides medication administration as applicable and may provide treatment in a variety of programs including, but not limited to rTMS or suboxone. The RN participates in performance improvement processes for Outpatient Mental Health.
