The Licensed Practical Nurse demonstrates technical proficiencies in initiating, performing and completing assigned duties in providing care to patient populations in the assigned Patient Aligned Care Team (PACT) or Ambulatory Care Clinic. He/she reports directly to assigned Nurse Manager and indirectly to the Associate Nurse Executive and the Associate Director of Patient Care Services. THIS POSITION WILL SUPPORT THE FEDERAL GOVERNMENT'S RESPONSE EFFORTS TO COVID-19. SELECTED APPLICANTS MUST BE ABLE TO START IMMEDIATELY. Duties performed include, but are not limited to the following: Covid Screening/Covid vaccinations/ Covid clinics Collects data and collaborates in the assessment of the patients health status; Participates in health teaching and counseling to promote, attain, and maintain the optimum health level of patients, as delegated; Establishes, maintains, and documents therapeutic relationships with patients, such as reality orientation and one to one and /or small group interactions; Participates in work groups, teams and/or committees and supports the facility performance improvement activities and organizational goals; Establishes constructive relationships with individual patients to elicit feelings and attitudes and to promote positive relationships, communications, and socialization skills; Assists with triage/intake; Completes clinical reminders; Provides personal care to patients and utilizes therapeutic approaches, which promote independence and self care. Work Schedule: Monday - Friday, 7:30 am - 4:00 p.m., may require other tours to meet patient care needs Financial Disclosure Report: Not required Physical Requirements: Heavy lifting (45 pounds & over), Heavy carrying (45 pounds & over), Straight pulling (1-2 hours), Pushing (1-2 hours); Walking (up to 8 hours), Standing (up to 8 hours), Kneeling, Repeated bending (up to 8 hours), use of fingers, both hands required; both legs required, Ability for rapid mental & muscular coordination simultaneously; Near Vision correctable at 13" to 16" to Jaeger 1 to 4,, Far Vision correctable in one eye to 20/20 and to 20/40 in the other, repetitive computer entry, ability to distinguish basic colors, Depth perception; Hearing (aide permitted); emotional/mental stability. Environmental Factors: Working closely with others, working with hands in water, working with machinery with moving parts, protracted or irregular hours of work; Use stretchers to transfer patients, use patient lifts, if applicable. May be exposed to infected patients and contaminated materials.
