This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Learn more The Registered Nurse (RN) Program Manager (PM) for the CLC works in close collaboration with the Chief Nurse CLC, guiding and directing the operations of the CLC to ensure administrative requirements and functions of the CLC are satisfactory. The incumbent overseas admissions to the CLC, Quality Assurance Performance Improvement (QAPI), staff education and resident/family satisfaction. He/she works in close, continuous collaboration with the CLC Chief Nurse to ensure the CLC Quality Assessment and Assurance (QAA) Committee meets its objectives according to standards and regulations to include but not limited to agenda development, yearly monitor grid, QAA Annual appraisal and coordination of monthly QAA meetings. The incumbent surveys/site visits action plans are completed timely and accurately and that data is analyzed to find patterns and trends making the necessary recommendations/ changes to improve performance. The RN/PM must ensure the care and services provided meet the satisfaction of the residents and their families, thus maintaining/increasing customer satisfaction. The incumbent functions as CLC Admission Screening Coordinator and performs but not limited to the following duties: Performs quality assurance and assessment, collecting, aggregating and analyzing data and presenting recommendations in the QAA Committee. Serves as support to the CLC staff responsible for all performance improvement monitors by serving as a resource for analysis to make significant recommendations. Serves as primary liaison with residents, their families and the general public. Serves as facilitator of the CLC Resident Council and Family Council Meetings. Works in close collaboration with the Patient Representative Office, CLC Nurse Managers and Interdisciplinary Team Members to assure all complaints/concerns regarding the CLC are resolved promptly and satisfactory. Aggregates and analyzes all data regarding CLC grievances/complaints/concerns to observe for patterns/trends and design/implement recommendations for improvement. Stays abreast of the State Operations Manual (SOM) for Long Term Care (LTC) and ensures the CLC meets the federal regulations for admissions, QAPI, education and resident rights/satisfaction Performs nursing screening of CLC consults to ensure candidate meets criteria for admission to the CLC. Ensures CLC staff has the competencies to provide the service the Veteran requires. Ensures staff receive the proper training and competency validation prior to accepting a Veteran that needs a service not commonly practiced in the CLC but currently performed in homes. Assures Veteran's medical and/or psychiatric conditions can be managed in the CLC by the nursing staff. Ensures CLC Admission Agreement with Veteran/next of kin. Assigns CLC bed as per Veteran's needs. Performs tracking of consults and aggregates data to perform analysis of consults received, accepted and denied and any other indicators to help improve the admissions process to the CLC and facilitate patient flow in acute care.
