The Registered Nurses in Nursing, GEC & R provide appropriate care for Veterans aged 70 and above taking into consideration diagnosis physical, psychosocial, spiritual, and cultural needs. The GeriPact Registered Nurse (RN) Care Manager is responsible for coordinating and managing the care of Geriatric Population in an outpatient clinic setting, which includes leadership in delivering and improving holistic care through collaborative strategies with others. The Registered Nurse Care Manager participates in quality improvement and evaluation processes related to the management of patient care. The Registered Nurse (RN) will provide assessments, health histories, telephone triage, care management, medication administration/counseling and patient/family education; special procedures, respite screening, exit interviews, Mini Mental Stats Exams/Saint Louis University Mental Status (MMSEs/SLUMS); and office operations. The Registered Nurse Care Manager will communicate with all members of the GeriPact team to promote continuity of care to assure that patients total care needs are met. The RN will lead the Licensed Practical Nurse (LPN) assigned to the GeriPact Clinic. NOTE: during the COVID 19 pandemic, all nursing staff are subject to be detailed to other areas of Nursing for COVID Patient Care coverage. Work Schedule: Monday through Friday, 8:00 a.m. - 4:30 p.m. (may include rotating weekends, evenings) Subject to change based on agency needs. Financial Disclosure Report: Not required
