The Nurse Practitioner (Intensive Community mental Health Recovery) will provide behavioral health care coordination for veterans at the Tyler Community Based Outpatient Clinic, Tyler Texas. Telework may be eligible for highly qualified candidate. The ICHM-Nurse Practitioner duties and responsibilities for the position includes but are not limited to the following: Work with serious and persistent mental illness Veterans in the rural health community, using a combination of telehealth and face to face clinic visits. Practice to the full extent of their education, training and certification, without the clinical supervision or mandatory collaboration of physicians as a licensed independent practitioner. Provide specified medical or other patient care service as the clinical provider who practices within the Rules and Regulation of the nursing practice Act of the state licensure board. Collaborate with other disciplines to develop, implement, and coordinate a system of patient carte delivery that promotes communication, teamwork, high quality care, patient satisfaction, positive outcomes, and cost-effective services. Recognize the complexity of care required in the psychiatric patient and can assist with developing a partnership to employ patient goals that may renew hope, involvement in meaningful activities, becoming empowered, overcoming social barriers to inclusion and managing symptoms. Assesses, plans, implements, and evaluates care based on age and cultural-specific components. Assumes responsibility for the coordination of care focused on Veteran transition through the continuum of care, Veteran, and family education, Veteran self-management after discharge, and supporting factors impacting customer satisfaction. Considers all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences. Administers medications and procedures per established policies and guidelines. Provides cross coverage to other areas as assigned by management. Influences care outcomes by collaborating with members of the interdisciplinary team. Actively participate in unit/program quality improvement processes and initiatives and customer service programs.
