We are looking for an Assistant Nurse Manager for Home Based Primary Care (HBPC) at the Salem VA Medical Center, who's a leader in delivering and improving holistic patient care and supervises nursing personnel and support staff. The Assistant Nurse Manager is directly accountable to the HBPC Nurse Manager, the Associate Chief, Nursing Service for Extended Care Service Line, the Associate Director, and Patient Care Services. Duties are but not limited to: Provides supervisor guidance and evaluates staff performance in collaboration with Nurse Manager and/or related program leader. Communicates/collaborates with Nurse Manager to assure compliance with standards of care and practice. Facilitates team approaches to identify, analyze and resolve problems. Identifies positive and negative aspects of patient care delivery and promotes patient-driven care in a safe, efficient and cost-effective manner, using evidence-based principles. Assures compliance of policies and procedures. Maintains current knowledge of multidisciplinary resources, programs and services, referring patients for community resources as appropriate. Assists nursing staff in assessing, planning, implementing and evaluating patient care in a cost effective and efficient manner. Demonstrates advanced clinical knowledge in assessing planning, implementing, documenting, and evaluating care for a designated group of patients across the continuum of care. Triages and applies a collaborative team approach in identifying, analyzing, and resolving patient care problems. Serves as a resource for patients and staff in addressing ethical issues; and promotes ethical conduct among staff. Assures detailed, current and comprehensive documentations are contained in the patient's medical and other pertinent records. Acts as a patient advocate when patient self-determination is in question; and advocates fiscal responsibility through monitoring utilization of resources. Manages nursing resources (financial, human, informational, and material based) to provide efficient and effective patient care delivery and staff management. Acts as a role model in fulfilling work commitments, seeking out and assuming additional responsibilities and assisting others with career development. Participates as a clinic leader with other program leaders in the decision-making process related to patient care, clinical/program policies and procedures, customer service and performance improvement activities. Demonstrates leadership in unit/service level quality improvement activities. Facilitates process improvement initiatives; identifies needs for improvement and utilizes innovative and creative approaches for changing practice based on improvement findings.
