You will serve as a Licensed Practical Nurse for the Care in the Community Service with the Overton Brooks VA Medical Center in Shreveport, LA. This position will support the Federal Government's response efforts to COVID-19. Applicants must be able to start immediately. The Licensed Practical Nurse functions as a part of the Community Care/Non-VA Care team in reviewing care in the community consults in accordance with VHA guidance in order to ensure the services requested and provided are appropriate, timely and cost effective. Serves as a care coordination liaison through frequent contacts via telephone or in person with all stakeholders involved with services rendered and performed in the community. Works under the direction of the Community Care Nurse Manager and Registered Nurse Care Coordinators. Work Schedule: Monday - Friday, 8 AM to 4:30 PM; OR 9 AM to 5:30 PM; OR 10 AM to 6:30 PM Financial Disclosure Report: Not required
