The Medicine Service RN Navigator provides a wide range of outpatient care coordination between the patient and caregivers for the Medicine Service. This role will work closely with Pulmonary, Sleep and Infectious Disease providers, Community Care staff and the Veteran/family to ensure the best patient care is provided. This role will be accountable primarily to the Nurse Managers, the Chief Nurse of Medical Service, and the Chief of Medicine.
