You Will Be Responsible For:
Performs assessment skills and formulates an individualized care plan based on therapeutic principles as appropriate for age/psychological development; evaluates the effectiveness of nursing/medical interventions and makes modifications as needed in accomplishing desired patient outcomes; assists in the assessment of the patient with Telestroke/Neurology as needed.
Navigates a diagnosed neurological patient from the admission diagnostic phase through the treatment and discharge phase, ensuring appropriate/timely medical/supportive services; good organizational skills in planning/implementing daily activities, and facilitates early discharge of patients.
Communicates effectively with physicians, health care team, patients and families to ensure coordinated and cohesive execution of evidence based quality patient care; effectively/consistently facilitates with members of the health care team to develop treatment plans, increase patient satisfaction, address specific patient concerns and discharge planning; participates in team conferences and provides information relating to therapeutic concerns in a clear, concise manner; reports to program manager problems or concerns involving direct/indirect patient care in a timely/professional manner; establishes/maintains a therapeutic relationship with patients /families.
Develops, coordinates and documents patient education and care. Serves as a primary ongoing resource for patients and families during their diagnosis and treatment period; assesses patientsâ™ learning style and health literacy in order to provide education on the appropriate patient level.
Assumes responsibility for professional growth and demeanor, attends all mandatory nursing/ hospital in-services, maintains and updates knowledge/skills of current trends, practices changes, and researches findings related to the cerebrovascular and neurologic disorder patient population and application to clinical practice; provides leadership through clinical mentorship and role modeling and collaborates with other health care team members to identify clinical practice models which optimize patient outcomes that are efficient and cost effective.
What Will You Need:
EDUCATION AND EXPERIENCE REQUIRED:
Associateâ™s degree in Nursing
Three yearsâ™ experience in neuroscience nursing
Experience with projects or program start-up
EDUCATION AND EXPERIENCE PREFERRED:
Experience with projects or program start-up
Bachelorâ™s or Masterâ™s degree in Nursing
Previous leadership experience
Teaching experience in formal/informal settings with knowledge of adult learning principles
Experience with neurology assessment
Experience with endovascular patient management
LICENSURE, CERTIFICATION, OR REGISTRATION REQUIRED:
Current, valid licensure with Florida State Board of Nursing as a Registered Professional nurse
Advanced Cardiovascular Life Support (ACLS) Certification
Basic Life Support (BLS) Certification
LICENSURE, CERTIFICATION, OR REGISTRATION PREFERRED:
Certification in Neuroscience Nursing (CNRN) or Critical Care Nursing (CCRN)
The Neuroscience Navigator facilitate an evidence-based, multidisciplinary, multi-specialty approach to neurologic care throughout the care continuum at the campus. Assesses the patientâ™s status and works with the physician and multidisciplinary team to determine appropriate plan of care for the patient. Provides direct clinical service to patients and families including care management and patient education to understand diagnosis, treatment and follow-up regimen. Provides education and mentoring to advance staff knowledge at the campus. Gathers, abstracts and reviews data for process and quality improvement. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
