Senior Services is a division of Fairview Home Care and Hospice that treats patients living in the Ebenezer assisted living, memory care and independent living communities. We began our Senior Services Rehab Program 3 years ago and nowsupport 24 plus assisted living communities throughout the Twin Cities Metro area in Minnesota. In these settings our staff get to know each other well and have strong team collaboration, which improves communication and collaboration and in turn brings higher satisfaction to our patients.
We are seeking at Lymphedema Occupational Therapist for our Senior Services Department.This role will support senior sites at various Ebenezer Facilities (LTC, Assisted living, TCU) with a caseload in the North Metro assisted living facilities. You will do general OT visits as well in the assisted living facilities as back up to other OT's assigned to those buildings. Our current North metro locations include; Roseville, Mounds View, White Bear Lake, Mahtomedi, Lake Elmo, Forest Lake, and Wyoming. Home/Base office is in Minneapolis off 26th Ave S.

The Edema Therapist- Occupational Therapist plans, organizes, coordinates and controls a caseload of home care OT patients, in addition to those patients requiring specialized edema therapy. Acts as a consultant to clinicians and provides education regarding edema and edema therapy.
Maintains an individualized plan of care, related to the patient's needs, in collaboration with the physician, patient and significant others. Assesses and evaluates the patient's edema and establishes an individualized Complete Decongestive Therapy (CDT) treatment plan, including ordering necessary supplies, in partnership with the patient's physician. Determines appropriate utilization of resources based on clinical assessment, HHRG guideline and standards of care.
Accountable for managing the clinical episode of care to achieve desired patient outcomes and measurable functional goals.
Communicates consistently and effectively with patient, caregiver, significant others, physicians and all team members to assure appropriate implementation and completion of the patient plan of care.
Maintains complete, accurate, and timely clinical records/ documentation on all patients according to agency policy. Completes documentation on the day of service.
Provides safe, effective skilled and advanced intervention, within scope of practice according to patient need. Provides counseling and teaching of patient, family and caregiver regarding care needs and health promotion/prevention. Provides patient and caregiver education in self-care management for effective edema therapy treatment. Evaluates and documents response to care/services delivered
Licensed as an Occupational Therapist and eligible to practice in Minnesota
Certified Lymphedema Therapist - certification in Complete Decongestive Therapy (CDT) Fairview requires 135 hours which includes head, neck, and trunk
Current CPR/BSL certification (Healthcare Providers)
Minimum Experience Required :
1 year recent clinical experience as an OTR in a rehab or acute care setting or home care.
Ability to function in job role independently
Previous home care experience
Valid Minnesota driver's license (or license that matches your current permanent residence).
Current and adequate automobile insurance policy
Computer skills: able to document effectively in EMR
