The Nurse Care Manager collaborates with members to build a Person Centered Service Plan (PCSP) that is based on a comprehensive assessment and is aimed to improve and maintain health, mobility and community participation. This is a fast-paced and exciting job!
Conduct comprehensive in-home assessments
Develops a Person Centered Service Plan (PCSP) with SMART interventions and goals
Works collaboratively with Care Coordinators and other team/staff members in monitoring and updating a panel of Member care plans
Identify and assist in developing resources and services as needed by members.
Respond to the needs of members and the organization, even if those needs do not coincide with the details of this job description
Associate's Degree in Nursing required; Bachelors preferred
Registered Nurse currently licensed to practice in NYS
Minimum 2-years Nursing Experience.
Comfortable with extensive travel in Brooklyn, Manhattan, the Bronx, and/or Queens
Experience working in a multi-site organization a plus.
Experience with working with in rehabilitation and/or with people with disabilities
Previous experience in non-profit organization preferred.
Experience with Medicaid Population
Bilingual (English/Spanish/Russian/Chinese/Any other language) a plus.
Telecommuting is allowed.
