We have positions at MedStar Washington Hospital Center in Washington, DC as well as MedStar Union Memorial Hospital in Baltimore, Maryland.
Ensure patients receive the right level of care at the right time
Work closely with world-class surgeons, referring physicians, patient families, and other health care professionals to promote a smooth, efficient, and customer-focused transition from hospital to home
What we’re looking for:
Bachelor’s Degree required
Master’s preferred in Social Work, counseling, vocational rehabilitation, or other qualified clinicians as defined by CMS
Valid Maryland license – Social Workers, LSWA, LGSW, or LCSW; Vocational Counselors: CRC, CCM, or CDMS
Three years of experience preferred
Demonstrated leadership skills
