ProHealth Home Care one of Wisconsin's most comprehensive sources of medical care and hospice care provided in the home. We provide a seamless continuum of care for people of all ages. Residential hospice care is offered at ProHealth AngelsGrace Hospice, a beautiful, 15-bed hospice nestled on eight scenic acres in Oconomowoc. ProHealth Care also offers quality in-home hospice care and in-home skilled nursing care. We are state-licensed, Medicare-certified and accredited by the Joint Commission.
Our highly skilled nurses provide a wide array of services, including medication education and management, injections and intravenous therapy, wound care, nutritional counseling, bowel and bladder management and much more. Physical, occupational and speech therapists address home safety concerns, strengthening and rehabilitation needs. Our licensed social workers offer assistance with community resources, financial assistance and supportive care and counseling to patients and families.
Hours: 7am-7pm Saturday and Sunday, 3 weekends per month, holiday rotation
Full Time / Part Time: Part Time with Benefits
The RN Intake Nurse coordinates the intake of new patients to the Home Health and Hospice agency. This individual assists in planning for services, responds to requests for information on home care and hospice services, triages calls coming into the office or answering service from patients, caregivers, medical providers or referral sources during business and non-business hours. The RN Intake Nurse will also function as a liaison with various inpatient facility discharge-planning departments, coordinating referrals to the home health agency.
Bachelors in Nursing preferred, Associates degree in Nursing required
Wisconsin RN License required
Minimum of 2 years nursing experience
Homecare experience preferred
